Dallas Service Delivery Area since 2018.

Transition Assistance Services
Dallas Elite TAS provides assistance to people, who reside in nursing facilities, who are Medicaid-eligible and who are able to return to a community living arrangement. The services provided include assistance moving to independent living housing, assisted living facilities and shared housing arrangements. Those who participate in the program May qualify for financial assistance through the Transition Assistance Services (TAS) grant and are enrolled in Star Plus Waiver Medicaid upon returning to the community.
The TAS Grant may be used to:
Assist with payment of security
depositsrequired to lease a home,
including an apartment, or to
establish utility services for a home.
Purchase of essential furnishings for a home, including a table, a bed, chairs, window blinds, eating utensils, and food preparation items.
Pay expenses required to move personal items, including furniture and clothing, into a home.
Relocation Services
Dallas Elite Transition Assistance Services provides relocation services to Superior Health Plan members exiting nursing facilities in the Dallas service delivery area (SDA). Dallas Elite Transition Assistance Services provides relocation services for clients who are long term care facility residents and who meet other specific criteria set aside by Texas Health and Human Services (HHS). The primary purpose of the relocation function is to support the transition of members and future members who desire to move from an institution to the community.

Our goal is to provide nursing facility residents, transitioning to the community, with the essential household items needed to furnish their home and with hope as they embark upon a new chapter in their life.
